This form should be filled out by a DZ Owner, DZ Manager, S&TA, Examiner or National Judge that was on-site July 13th, 2024. Please submit this form within 48 hours of the completion of your jump day.

Number of skydives completed on July 13th, 2024 Count the number of people, a tandem is TWO people. For experienced skydivers, count the number of jumps made by each person.

Ex:
Lina - licensed jumper who made four jumps = 4

Joe - Tandem Instructor who made five tandem jumps = 5

Joe's five tandem students who made one jump each with Joe = 5
Total = 14 skydives completed
