In order to verify your skydive(s) and send you an official certificate, we need a form of verification. Please send us ONE of the following:  
  
Tandem Skydives: 

• Jump certificate dated July 12th, 2025 signed by your instructor  

or  

• Dated receipt which reflects your jump on July 12th, 2025

Student or Licensed Skydiver: 

• Logbook pages showing jumps made on July 12th, 2025 completed and signed appropriately  

or  

• Screen grabs of Burble (or other manifest software) transaction page showing jumps made on July 12th, 2025  

or  

• Screen grabs of electronic logbook entry appropriately signed and dated July 12th, 2025
