In order to verify your skydive(s) and send you an official certificate, we need a form of verification. Please send us ONE of the following: 
 
Tandem Skydives:

Jump certificate dated July 12th, 2025 signed by your instructor 
or

Dated receipt which reflects your jump on July 12th, 2025
